I, the undersigned parent/guardian of the guest named above, give my full consent for my child to attend the 18th birthday celebration of Borgy Jazz Mindajao on December 6–7, held at The Summer House, Sogod, Cebu.

I understand and acknowledge the following:

A van will transport the guests from Insular Square, Mandaue on December 6.

Guests must arrive before 11:30 AM.

The group will depart at 11:45 AM sharp.

The event includes access to a beach and private pool, and my child may bring swimwear and a towel.

Adults, including Xane Andrino and Benjo Andrino, will supervise the event.

Transportation back to the same meet-up point will be provided on December 7.
