CEU Event Accreditation Form

Baltimore-Washington Conference of the UMC

Board of Ordained Ministry

This form is to be used for registering an event or course offered in the Baltimore-Washington Conference where CEUs will be awarded.

Event organizers must provide a CEU certificate or letter of completion to participants. The certificate should include:

• Name of the participant;
• Lecture, Program, or Training title;
• Date course was presented;
• The name of the organization providing the course (if applicable);
• CEUs awarded (1 contact hour = 0.1 CEUs)
• Signature of event organizer
• A line that reads, "Approved for Continuing Education by the Baltimore-Washington Conference Board of Ordained Ministry"

Also, please let BWC participants know up front that they will need to submit this CEU to the Conference through the CEU submission tab in their Arena account.
